Risultati a lungo follow-up della ricostruzione artroscopica del legamento crociato anteriore: tendine rotuleo versus tendine semitendinoso e gracile

Long term follow-up results in anterior cruciate ligament reconstruction: bone patellar tendon bone compared with double semitendinosus and gracilis tendon graft

Riassunto

Obiettivo

Obiettivo dello studio è stato valutare le differenze nel recupero postoperatorio, dopo ricostruzione del LCA utilizzando, come trapianto i tendini del semitendinoso e gracile raddoppiati (STGD) versus il tendine rotuleo (BPTB).

Materiale e Metodo

160 soggetti sottoposti ad intervento di ricostruzione del LCA tra gennaio 1998 e gennaio 2007, sono stati valutati al follow-up compreso tra 1 e 10 anni (in media 5). I soggetti erano stati operati in maniera random con uno dei due trapianti. I 160 soggetti sono stati divisi in due gruppi di 80 ciascuno. La ricostruzione del LCA è stata eseguita, nel gruppo A, con il terzo centrale del tendine rotuleo e nel gruppo B, con STGD. Il gruppo A era composto da 47 maschi e 33 femmine, con età compresa fra i 19 e i 49 anni (media: 27,7 anni). Il gruppo B era composto da 51 maschi e 29 femmine, con età compresa fra i 19 e i 41 anni (media: 25,5 anni). La valutazione dei risultati clinici al follow-up è stata eseguita mediante la scheda IKDC (International Knee Documentation Committee) e la valutazione radiologica con rx in AP ed LL del ginocchio sotto carico ed RMN.

Risultati

I risultati clinici relativi a 160 casi, ottenuti mediante valutazione con scheda IKDC, non hanno mostrato differenze statisticamente significative tra i due gruppi ed hanno evidenziato in oltre il 90% dei casi, risultati normali o quasi normali in entrambi i gruppi. La valutazione soggettiva ha mostrato una, seppur lieve, minor incidenza di dolore e sintomi nel gruppo trattato con STGD ed una maggiore incidenza del fastidio all’inginocchiamento, nel gruppo BPTB. I risultati di imaging hanno evidenziato inoltre, che quando la lesione del LCA è associata a lesioni condrali e meniscali, si nota al follow-up un peggioramento del quadro radiologico, soprattutto se l’intervento di ricostruzione del LCA è avvenuto dopo molto tempo dal trauma.

Conclusioni

La ricostruzione artroscopica del legamento crociato anteriore può essere attuata con entrambe le tecniche prese in esame; l’utilizzo del semitendinoso e gracile sarebbe comunque da preferire, perché evita le problematiche connesse al prelievo del tendine rotuleo. La ricostruzione con innesto tendineo, comunque, non riesce a riprodurre completamente la complessa biomeccanica del ginocchio e, pur conferendo stabilità e propriocezione all’articolazione, non è in grado di prevenire l’insorgenza di alterazioni degenerative, soprattutto in pazienti con associate lesioni meniscali e condrali, nonostante il trattamento contestuale di queste ultime. Nella prevenzione dell’artrosi più del singolo intervento di ricostruzione, riveste un ruolo fondamentale il mantenimento dell’omeostasi articolare.

 

Summary

Aim of the study

The purpose of the study was to evaluate the differences in post-operative results of ACL reconstruction using as graft semitendinosus and gracil tendons or bone patellar tendon.

Materials and Methods

160 patients who underwent ACL reconstruction between January 1998 and January 2007, in our department were analyzed at the followup from 1 to 10 years (mean follow-up 5 years). These patients were treated using random one of the two grafts. They were divided into two groups of 80 patients each. In group A we used the bone patellar tendon bone (BPTB), while in group B we used the semitendinosus and gracilis tendon graft (STGD). In group A there were 47 men and 33 females, aged from 19 to 49 years (mean age: 27years 7months), in second group there were 51 men and 29 females, aged from 19 to 41 years (mean age: 25 years 5 months). To evaluate this study we used the International Knee Documentation Committee (IKDC), AP and LL x-rays and MRI of the knee.

Results

There was not a significant statistical difference between the two groups. Clinical results were normal or almost normal in more than 90% of cases. In the subjective evaluation there was a smaller incidence of pain and symptoms into the second group and an higher incidence of pain to kneel into the first group. Imaging results showed that when LCA injury is associated to condral or meniscal damage, there is a worsening of x-ray features above all if the LCA reconstruction has been performed a long time after injury, even if condral or meniscal lesions are treated in the same surgery.

Conclusion

Arthroscopic LCA can be performed using both grafts. STGD graft is preferable because it seems to avoid patellar tendon graft pathology. The ACL reconstruction with graft cannot reproduce completely the complex biomechanics of the knee so, even if it gives stability to the knee joint, it cannot prevent degenerative changes.
The ACL rupture associated to cartilaginous or meniscal injury promotes the arthrosis. For the prevention of osteoarthrosis it is important to preserve the homeostasis of the joint more than single ACL reconstruction with graft.
